AvaCare Medical offers the ability to place orders using Purchase Orders. This program is offered to government entities, medical facilities and nonprofit organizations, as well as university, college and school systems.
Creating an Account
To initiate a Purchase Order, follow these directions to create a corporate account with AvaCare Medical.
- 1. Download the appropriate credit application below
- 2. Send the completed form to email@example.com
- 3. If your application is approved, we will respond within 1 business day with a link to create your account

Placing a Purchase Order
Once your account is set up, you can place Purchase Orders at your convenience by following these simple steps.
- 1. Log into your account
- 2. Add items to your cart as usual and head to checkout
- 3. Select “Purchase Order” as your payment method
- 4. Enter your internal PO number in the appropriate field

The terms are NET 30 from the invoice date.
